Open lecture: Learning from Errors and Events

The objective of the session is to learn from errors and events. Looking at other industries gives an insight as to how badly stuff can go wrong with minimal deliberate intentions. The identification of the elements indicates that some items may be initially considered as "latent" but actually, as much of what we do is repetitive, we can predict where the errors will take place. The problem is stuff doesn't always get updated and changed.
